If you need a 911 address assigned, please complete this form this form and return it to the office address shown on the form.

Addressing Fees:

Individual Residential Addressing Fee:
$100.00 per address
Adding Subdivision Streets Fee:
$250.00 per street for up to 10 streets per phase
Flat rate of $2,500 per phase for additional streets
Maximum Development Cost of Subdivision Phase:
$4,000 per subdivision phase will be assessed
Governmental agencies and chartered non-profit organizations are exempt from these charges but must still adhere to the process for addressing.
